Cashless facility for Hospitalization

Call the assistance number, provide certificate number & insured name. Specify your geographic location. The assistance will guide you to the nearest hospital.

After hospitalization insured/or someone on behalf of the insured calls the assistance number. Provide the certificate number, insured name and hospital details* to the assistance executive.

Hospital Claim

GOP (guarantee of payment) is placed by the assistance company to the hospital. Direct settlement will be done with the hospital for expenses under the covered benefits

* Name of the Hospital & location'/'Address'/'Telephone number of the hospital'/'treating doctors name'/'medical expenses incurred'/'paid and review appointment details.

Note:
  • If you have a hospital of your choice - please check with the assistance, if cashless facility can be arranged with the hospital of your choice
  • GOP may take 24 hours or less after intimating the assistance toll free
  • Settlement will be as per the covered benefit subject to policy terms and conditions.

OPD Treatments less than $500 & OPD Treatments above $500
  • Travel Companion plan:  For outpatient consultation(s), the Insured will have to self-pay and file the claim upon return to India, provided the medical condition is confirmed to be non-pre-existing.
  • Bajaj Allianz travel Elite plan:  For outpatient consultation(s), the Insured will have to self-pay and file the claim upon return to India. However, if the amount is above USD 500 we can arrange to make payments on behalf of the patient, provided the medical condition is confirmed to be non-pre-existing.
  • Travel AssistCard plan:  Completely cashless both for inpatient and outpatient medical expenses. It covers for pre-existing conditions as per the policy terms and conditions.
    Please note :
    • The Insurance medical assistance department (doctor) will discuss your medical condition with the treating doctor and if it is confirmed that the admission to a hospital is NOT due to any pre-existing conditions or any conditions listed in the exclusion list, then the Insurance Company shall guarantee payments to the hospital and settle the payments with the hospital.
    • For medical sickness/accident there is a policy excess of 100 USD (deductible) which the Insured will have to self-pay and this amount cannot be claimed. This implies for any claim the first 100 USD are to be borne by the insured. This applies for both outpatient and inpatient treatment.

  6. Copy of passport/Visa with Entry & exit stamp
  1. Please contact our Assistance Company at the number given above and obtain a Claim Form from them and fill in. It is necessary to obtain the attending physician's signature on the Form.
  2. Please collect all bills/receipts/invoices
  3. Send all documents to Bajaj Allianz, Bangalore.(Refer to detailed steps on Accident & Medical sickness claims)
AD & D (Common Carrier)
  1. Our Claims Dept. will advise on documentation upon receipt of Claim notification.
  1. Collect all documents pertaining to the loss including correspondence with carrier and submit all documents to the address mentioned on the policy schedule.
  2. Claim form can be obtained from our Assistance Dept.
Hijack
  1. Covering letter detailing the incident
  2. Duly filled & signed claim form
  3. Passport/Visa copy with entry & exit stamps
  4. Boarding Pass and Airline Tickets
  5. The certificate issued by Airline indicating the Hijack & the total duration of Hijack
  6. Any other document as required would be advised later if required.
  1. Inform claims Department immediately at the contact number given on the policy schedule, giving full details of the incident
  2. Submit the claim documents to the address given on the policy schedule.
Loss of Baggage
  1. Claim form
  2. Copy of the passport/Visa with Entry & exit stamp
  3. Copies of boarding pass/Ticket/ Baggage Tags
  4. Copies of Correspondence with the Airline authorities/Others about loss of checked baggage
  5. PIR report (To be obtained from the Airline authorities)
  6. Details of compensation received from Airlines/Other authorities
  1. Intimate the airline about your loss and lodge complaints.
  2. Claim Form can be obtained from our Assistance Company or IBM Intranet.
  3. Fill in the Claim Form and send all documents to Bajaj Allianz, Bangalore.
Loss of Passport
  1. Copy of new passport
  2. Copy of previous passport (if available)
  3. Copy of return tickets
  4. Original bills/invoices of expenses incurred for obtaining a new passport
  5. Copy of FIR/ Police Report
  1. Intimate the airline about your loss and lodge complaints.
  2. Claim Form can be obtained from our Assistance Company or IBM Intranet.
  3. Fill in the Claim Form and send all documents to Bajaj Allianz, Bangalore.
Delay of Baggage
  1. Claim form
  2. Copy of the passport/Visa with Entry & exit stamp
  3. Copies of boarding pass/Ticket/baggage tags
  4. Copies of Correspondence with the Airline authorities certifying about the delay
  5. PIR report (To be obtained from the Airline authorities)
  6. Original bills/receipts/invoices pertaining to expenses incurred/purchases made during the delay period
  7. Details of compensation received from Airlines/Other authorities
  1. Obtain confirmation of the delay from the airline
  2. Claim Form can be obtained from our Assistance Company.
  3. Fill in the Claim Form and send all documents to our Bajaj Allianz
Personal Liability
  1. Full statement of the facts in writing
  2. Witness statements
  3. Any other documents relevant to the incident, including Summons, Legal Notice etc.
  1. Inform our Corporate Claims Dept. immediately (at the address given below) giving full details of the incident.
  2. Do not commit any benefit/compensation or enter into any agreement.
Trip Delay
  1. Claim form
  2. Copy of onward & return tickets
  3. Communication from the Airlines confirming the duration of delay and the reason for delay.
  1. Obtain confirmation of the delay from the airline
  2. Claim Form can be obtained from our Assistance Dept.
  3. Fill in the Claim Form and send all documents to our Bajaj Allianz.

* Note: We may call for additional documents/ information as relevant.

** If any hospital does not submit a bill to you for the treatment/service rendered, please intimate our Service Center before you leave the hospital.

  1. Always carry insurance certificate issued by your travel desk before traveling, please check with your travel desk if your insurance certificate is validated for your expected travel days.
  2. Please call the Toll Free numbers mentioned on your card / policy certificate before admission to any hospital (some extra numbers for a few more countries are printed on this document)
  3. Deductible is the payment that will have to be made by insured to the hospital directly for any claim made.
    Example:

    If there is hospitalization claim for $1000 while you are in America, Bajaj Allianz will pay the hospital $900 Employee will have to pay $100

  4. Coverage is for a maximum duration of 180 days per single trip and can be renewed for another 180 days with approval from the underwriting team.
  5. Inpatient Cashless above $100
  6. Cashless outpatient bill settlement depending the plan you purchase
  7. Routine medical checks not covered
  8. Pharmacy Drugs - pay and claim

  10. If there is a requirement for an extension due to any reasons, please ensure that your Travel Desk is informed prior to the end date of the initial itinerary.
